RAPID CITY, S.D. — Sanford Health is expanding its footprint in the Black Hills with the acquisition of Creekside Medical Clinic, a long-standing, physician-owned primary care provider in Rapid City.
The Sioux Falls-based health system announced Monday that Creekside will officially join the Sanford network on Sept. 1, 2025. The clinic, located on Jackson Boulevard, will retain all existing physicians, providers, and staff.
“Sanford Health has a long history of bringing together local, independent physician groups that share our vision for the future of health care,” said Kyle Wiese, president and CEO of Sanford Rapid City. “We are excited to welcome this team of talented primary care physicians to Sanford Health.”
The move continues Sanford’s aggressive expansion strategy in western South Dakota. In late 2024, the healthcare powerhouse acquired Black Hills Surgical Hospital, Black Hills Orthopedic and Spine Center, Black Hills Urgent Care, and related entities in both South Dakota and Wyoming.
Sanford President and CEO Bill Gassen, a Rapid City native, said the acquisitions are part of a broader goal to enhance access and streamline care in South Dakota’s second-largest health market.
Creekside Medical Clinic has provided primary and preventive care in the region for more than 25 years. Its services include outpatient care for all ages, preventive screenings, vaccinations, physicals, and on-site lab testing. The clinic is also known for offering employment and DOT physicals.

With this acquisition, Sanford Health continues to consolidate healthcare services in the Rapid City area, positioning itself as a dominant force in the western South Dakota medical landscape.
