RAPID CITY, S.D. — Sanford Health has completed its combination with Creekside Medical Clinic, a long-standing independent, physician-owned practice in Rapid City that provides primary care to the Black Hills region. The deal closed Aug. 29.
Creekside Medical Clinic has served the community for more than 25 years with four family medicine physicians and two advanced practice providers. All staff members have joined Sanford Health, which operates the largest rural health system in the country. The clinic, located at 2822 Jackson Blvd., Suite 101, will continue offering outpatient care for children and adults, preventive health screenings, on-site lab work, vaccinations, and pre-employment and DOT physicals.
Kyle Wiese, president and CEO of Sanford Black Hills, said the move strengthens Sanford’s commitment to expanding access to care in western South Dakota.
“Sanford Health has a long history of bringing together local, independent physician groups that share our physician-led vision for the future of health care, and we are excited to welcome this team of talented primary care providers to Sanford Health,” Wiese said in a release.
The combination follows Sanford’s November 2024 integration with Black Hills Orthopedic and Spine Center, Black Hills Surgical Hospital, Black Hills Urgent Care and Black Hills Surgery Center of Wyoming. Leaders say the goal is to give patients easier access to Sanford’s network of specialty services, clinical trials and precision medicine.
Sanford has provided specialty outreach in Rapid City for more than 20 years, including pediatrics, fertility and genetic counseling, and maternal fetal medicine. The health system also operates a specialty reference laboratory in Rapid City and senior services at Good Samaritan’s St. Martin Village, Echo Ridge, New Underwood and Home Health of the Black Hills.
Headquartered in Sioux Falls, Sanford Health serves more than 2 million patients across the upper Midwest with 56 hospitals, 288 clinics and 147 senior care communities.
